What is your new patient procedure?

New patients can bring in their filled-in and signed new patient forms and complete registration upon arrival during their first visit. These forms will be emailed to you or sent via First-Class Mail. Please complete the forms and print them out one-sided to be scanned in. By filling out the registration forms before coming in, you will help streamline the registration process.

How much will my copayment be?

Your copay (also called a copayment) will vary depending on the service you receive and your health insurance plan, but copays are typically $30 or less. Copays are a form of cost sharing. Insurance companies use them as a way for customers to split the cost of paying for health care. Copays for a particular insurance plan are set by the insurer, so please be sure to speak to your insurance company to find out exactly what your copayment amount will be.

I don’t have my insurance card with me, can I still receive treatment?

Yes, you still have access to care. You do not need to have a physical member ID card to get care from your doctor or other health care professional. We can simply look up your eligibility and member ID card using our many online provider tools.
